The original sentence is perfect and impeccable in that what had been accompanied must have been the explorer, not the country Puerto Rico. Yes, the positioning of a phrase could cause confusion, but as far as this sentence is concerned, there should be no problem whatsoever.

2. He travelled around the world (accompanied by his saxophone). (Perfect!)
3. He travelled around the world, accompanied by his saxophone. (Perfect!)
4. He, accompanied by his saxophone, travelled around the world. (Perfect!)
5. Accompanied by his saxophone, he travelled around the world. (Perfect!)
We cannot say that "accompanied by his saxophone" qualifies "the world" simply because the former is positioned immediately after the latter (in Sentnces 1 and 2). It doesn't hold logical water.
